Quick Entry Menu
This allows calls to be responded to on-line. Type in the call # of the Incident Call Report you want to respond to and click on Call Response Entry Form.

 

Location/Call Summary Report

The first option on this report lets you choose what date range you want to see and whether you want to see all the Locations in your group or one in particular.

When the report appears you will see the Location Name, Start Date, Number of Power Units (Vehicles), Trailers (if applicable), Decal Information Form/Last Update, and either a date that the form was received or an indication that it has not been returned. Next, you will see a summary of all the Incident Call Reports received within that date range.

They will be listed as:


 

Call # Date/Time Decal # Driver Name Driver # Vehicle # Comment Incident

An Incident Report that has been responded to will have "Response Form" underneath the "Date/Time"and can be clicked on to be viewed.

Clicking on any category will produce the history of the information in that category.

 

Only show locations that have calls
This will list only locations that have received Incident Call Reports in a chosen date range.

Summary only
No call detail lines - this will list your locations, the number of Incident Call Reports, the number and percentage that have been responded to, and the number of incidents. (There can be more than one incident per Incident Call Report, i.e. Unsafe Lane Change and Tailgating, etc.)

Call Summary Report by Driver's Name/Number
This report will allow you to track the Incident Call Report history of a driver, either by Driver Name and/or Driver Number, or summary by vehicle number.

Call Summary by Decal Number
This report will allow you to track the Incident Call Report history of any decal number in the date range chosen.

Call Summary by Comment
Allows you to track Incident Call History by information stored in the "Comment" category of the Decal Information Form.

 

Decal Information Entry Form Return Report (Manifest Report)
This report is designed to track the return of the Decal Information Entry Form confirming the placement of the decals on the vehicles. The report can be queried to produce a report that will list all the locations, the date the decals were shipped, and either the date the Form was returned or if in fact it has not been returned. You can also produce a report that will just list the locations that have not returned the Form and the decal shipment date.

 

Additional Decal Request Form
Allows you to order additional or replacement decals online.

 

Decal Change Form
Allows you to add, delete or transfer decals within locations.

 

Multiple Call by Driver Name Report
This report will list the Driver's Names, in descending order, based on the number of calls received. Click on a Driver's Name to view the actual Incident Call Report history.

 

Time of Day Trend Report
This report displays a 24 hour graph and lists the number and percentage of calls based on the time the Incident occurred.

 

Incident Call Type Trend Report
This report lists The Call Types received (Unsafe Lane Change, Tailgating, etc.) by percentage, in descending order.

 

Actions Taken Trend Report
This report lists the percentage of Actions Taken by management in response to the Incident Call reports (i.e. Verbal Warning 26%, Additional Training 18%, etc.)

 

Response Forms Completed Report
This report allows you to view all Incident Call Reports, those that have been responded to, or those that have not been responded to.
